a) Personal particulars form (resume / curriculum vitae on qualifications and experience)
b) A passport-sized color photos (not returnable)
c) Copies of academic certificates and transcripts, identification card, birth certificate, latest salary slip and other related documents if any.

For candidates who are currently serving with the Government, Statutory Bodies and Local Authorities, the application shall be made by the Head of Department and attach a copy of the updated Service Statement and Annual Performance Evaluation Report attached.
